Are you a person with a disability interested in the arts, either as an artist or as an arts-appreciator? Are you an arts organization interested in increasing the participation of people with disabilities in your programming? Or maybe you represent a non-arts organization (like a social service organization or neighborhood association) interested in providing accessible creative opportunities for people with disabilities in your community? 

As we are developing the new accessibility grant program we need your input! There are two versions of the survey:

  1. A survey for PEOPLE WITH DISABILITIES – Specifically designed to get the input of individuals who identify as a person with a disability or disabilities (PWD); and
  2. A survey for ORGANIZATIONS – Designed to get the input of arts and non-arts organizations who want to grow the participation of people with disabilities (PWD) in their communities.

Are you BOTH a person with disabilities and a representative of an organization? Then feel free to fill out both surveys or just the one that speaks to you the most.
